Need a permission form? Generate one through EntryEeze.
The Club Permission Form is required for entering competition events and/or test sessions that are not hosted by Wagon Wheel FSC.
To access your permission form, click ACCESS
MY ACCOUNT, which will link you to the page for Existing Wagon Wheel FSC Members. You will then need to login with your email address and password you created when signing up for EntryEeze.
Select the desired skater and then the “Get a permission form” button.
Note: Permission Form letters are only valid for the current or renewed season.
